The Pain Management and Anti-Aging Center is located in Professional Park West Plaza, just west of Jefferson Pointe Mall and Apple Glen Crossing. Our address is 4656 W. Jefferson Blvd., Suite 250

Directions from I-69:

From the Interstate, take exit 102. This is Jefferson Blvd. You will need to turn toward Fort Wayne, or East. (From the North this is left. From the South, go right.) Continue down Jefferson Blvd. for approximately 3.5 miles. The Pain Management and Anti-Aging Center will be on your left, in Professional Park West. If you come to the stoplights for Jefferson Pointe Mall, you've gone too far. Inside Professional Park West, take the second left. The building will be on the right side of this parking lot.

Directions from downtown:

Start out heading west on Washington Blvd,which will merge into Jefferson Blvd. Follow this road away from the downtown area and toward the Jefferson Pointe Mall. You will pass Illinois Rd., the Jefferson Pointe Mall, and Apple Glen Crossing, all on the right. Immediately after, the entrance to Professional Park West will also be on the right, just behind Best Buy. Inside Professional Park West, take the second left. The building will be on the right side of this parking lot.
